Reach Kidney Care in Albany, Georgia is presently following 234 CKD patients and in an effort to reach out to the community we participated in a community outreach program at Henderson Community Center on May 30, 2015. Other resources and vendors present were Senior Resource Center, Georgia Families, Allergy and Asthma Clinics of Georgia, Community Action Council, Albany Technical College, Sam’s Club, and Helping Hands.
We had a booth set up to represent Reach Kidney Care. We provided blood pressure screenings, weights, and body mass index information. We educated on how diabetes and hypertension affect the kidneys, the importance of controlling the disease process, the importance of adhering to prescribed medications, and following up with appointments with health care providers. We also distributed pocket cards with information on “What You Need to Know” about chronic kidney disease.
It was a very rewarding experience to be able to participate in this community based project.
